Existing Skywarn™ Storm Spotters: Use the following form to notify us of any changes to your contact information. If your browser does not support forms, send an e-mail to Angela Enyedi, and try to answer as many questions from the online form as you can.
1. Personal Information   Enter your name, address, and reference location.

2. Skywarn Information   Enter your spotter ID, HAM radio ID (if applicable), and the date of your last spotter class.

 Date of last spotter
 class attended:
(Example: 12/5/02)
 Basic Spotter    Advanced Spotter

3. Contact Phone Numbers   Please enter the phone numbers at which you can be reached (including area code), along with whether or not we can call you during severe weather at this number, and the times that we can call. Remember, we would prefer that you call us during severe weather at (800)499-1594.

4. Submit Your Report   Click on "Submit Info" to send your updated spotter info to the NWS in Jacksonville. If you wish to start over, click on "Reset Form".
